MULTIPURPOSE SENIOR SERVICE PROGRAM
Providing Care at Home to the Frail Elderly
The Multipurpose Senior Service Program is a voluntary care management service. The program provides care management, which consists of providing information and assistance regarding appropriate community resources to meet needs, as well as monitor their on-going care and health status. A care manager and a public health nurse visit the client in his or her home, meet with family members if appropriate, and work with the individual to create a plan of care so they can remain at home. On-going and frequent contact between MSSP staff program participants ensures that the necessary care is provided.
Who Qualifies for MSSP?
Resident of El Dorado County
- Age 65 or older
- Receive Medi-Cal under an appropriate aid code
- At risk of being placed in a skilled nursing facility or intermediate care facility due to frail health
- Appropriate for care management services
- Voluntarily accepts MSSP services
The MSSP enrollment plan begins with a comprehensive lifestyle evaluation. After assessing the client's needs, the care team works closely with the individual to determine what they need to remain safely at home. The team provides information and assistance to the client and family and helps identify and coordinate a broad range of services which may include:
- In-home support services
- Counseling
- Respite care
- Adult Day Care
- Transportation
- Emergency response services
- Supplies & equipment
